Apply for the Wisconsin Shares Program at dcf.wisconsin.gov/wishares. Let us know how we can help with your application process.
Also, the City of Madison Child Care Assistance Program has limited funds available for new families, and no wait list. We encourage our families who may have need and who are ineligible for WI Shares Child Care Subsidy to apply. Families must reside in the City of Madison. Families can call (608) 267-4996 or email: childcareassistance@cityofmadison.com to complete a pre-application screening. For this call please have your financial information (wages, work hours etc.) on-hand. Once funds are allocated, a waitlist will be started.
Also, UW-Madison Students can apply for the Child Care Tuition Assistance Program (CCTAP) at Office of Child Care and Family Resources.
When funds allow, UHP extends scholarship funds to families demonstrating financial need and commitment to the UHP community. The number of scholarships available in a given year may vary with enrollment and number of applicants.
Families should first explore eligibility for WI Shares, City of Madison Child Care Tuition Assistance, and UW CCTAP before applying for UHP scholarship funds. Receipt of assistance from these programs does not automatically disqualify a family from receiving UHP funds, however, the amount awarded will be reduced accordingly. Amount awarded to any family will be capped at 50% total cost of tuition.
Please complete this form and submit it to UHP’s Parent Board Treasurer via email at treasurer@uhpparentcooperative.org. We will contact you with a decision and more information within 10-14 business days. If you need assistance filling out this form or would rather speak with someone to complete this application, please email the UHP Treasurer at the email address above or call the preschool office at (608) 238-3955.
